Form 1 - Application for approval of draft formation disclosure statement for a proposed co-operative

Fill out this form to confirm that a distributing co-operative's disclosure statement contains enough information for new members to fully understand their involvement. Non-distributing co-operatives don’t need to provide a disclosure statement unless directed by the Registrar of the Office Fair Trading. This form is effective from 1 December 2020.
