Template—Veterinary certificate for a regulated procedure on an animal

The Animal Care and Protection Act 2001 regulates certain procedures that can be performed on animals. The Act also requires a veterinary surgeons’ certificate to accompany an animal throughout its life if it has had a regulated procedure.

This template certificate has been prepared by the Department of Agriculture and Fisheries for use by veterinary surgeons that have completed a regulated procedure on an animal.

This template certificate is an example only. Veterinary surgeons may adapt the template to suit their requirements.
