Application to add details of parentage after registration of birth

This form relates to transitional provisions for the Surrogacy Act 2010 and is to be used to apply to alter or add parentage details as a result of amendments to the Status of Children Act 1978. Please note, to add father’s details to a birth registration, use the Birth registration application (Form 1).
