Chapter 4.9 - Enduring power of attorney sample
An enduring power of attorney (EPA) is a legal document which allows a person to appoint a person they trust (the attorney) to make decisions on their behalf about personal (including health) matters and/or financial matters and must be witnessed by a qualified witness.
