An enduring power of attorney (EPA) is a legal document which allows a person to appoint a person they trust (the attorney) to make decisions on their behalf about personal (including health) matters and/or financial matters and must be witnessed by a qualified witness.

How you do this is described in Chapter 4.9 - Witnessing general powers of attorney, enduring powers of attorney and advance health directives of the Duties of Justices of the Peace (Qualified) and Commissioners for Declarations handbook.
