Replacement blue/exemption card
Complete this form if you did not receive your blue or exemption card in the mail.
You can request a replacement card using the online applicant portal if you have a lost, stolen or damaged card.
Paper applications which are valid for lodgement until 30 June 2024 will continue to be accepted until further notice.
You can contact us and request this form be sent to you, or you can download, complete and submit a form by:
- scan and upload
- post, or
- delivering it in person.
Completing a paper form requires you either:
- verify your identity online and include your account number on the paper form
- provide a certified copy of your ID document.
