Exemption card business application (EB)

This form is to be completed by Queensland registered teachers and police officers proposing to start or continue in child-related employment. You can also do this using the online applicant portal.

Paper applications which are valid for lodgement until 30 June 2024 will continue to be accepted until further notice.

You can contact us and request this form be sent to you, or you can download, complete and submit a form by:

• scan and upload
• post, or
• delivering it in person.

Completing a paper form requires you either:

• verify your identity online and include your account number on the paper form
• provide a certified copy of your ID document.
