Eligibility declaration application (ELG)

This form is for disqualified people to check whether they are eligible to apply for a blue card.

Paper applications which are valid for lodgement until 30 June 2024 will continue to be accepted until September 2024.

You can contact us and request this form be sent to you, or you can download, complete and submit a form by:

• scan and upload
• post, or
• delivering it in person.

Completing a paper form requires you either:

• verify your identity online and include your account number on the paper form
• provide a certified copy of your ID document.
