Fill out this form to: convert to a co-operative; transfer the incorporation of your co-operative; acquire or dispose of certain assets; change the maximum permissible level of share interest in the co-operative; merge with or take over another co-operative; or voluntarily close down the co-operative. This form is effective from 1 December 2020.
