Approval to contact Blue Card Services

Use this form if you want to approve someone else (an approved person) to contact Blue Card Services (BCS) on your behalf or if you need help from a Blue Card Liaison Officer.

This form allows a Blue Card Liaison Officer or an approved person to:

• help make an application
• check with BCS for updates about your application
• provide information to BCS on your behalf (excluding confidential information)
• discuss your information with BCS, including some personal information required for the application (excluding confidential information).

This form does not give approval to discuss any confidential information, such as your criminal history.

If you want someone else to discuss your confidential information with BCS, you will need to complete the Approval to discuss confidential information with BCS form instead.
