Acknowledgement of sex Queensland born or adopted adult (16 or older) application (Form 20A)
On 24 June 2024, new laws started that affect changing the sex recorded on your Queensland birth certificate if you are 16 or older.
You are able to fill-in an application and pay online, visit www.qld.gov.au/AOSadult.
Use this form if you are unable to fill in the application form online to alter your recorded sex on your birth certificate.
