Acknowledgement of sex Queensland born or adopted child (under 16) application (Form 20B)
Making a request after 23 June 2024
On 24 June 2024, new laws start that affect changing the sex recorded on your child's Queensland birth certificate if they are under 16.
You will be able to fill-in an application and pay online, visit www.qld.gov.au/AOSchild.
Use this form if you are unable to fill in the application form online to alter your child's recorded sex on their birth certificate.
